Having acquainted myself with the rules, I/we have completed this application and am enclosing my entrance fee of $115 or $125 (check or money order) payable to: Bainbridge Bass Club. In signing this application, I/we hereby waive and release all other contestants, the host sponsors and tournament officials from all claims or injury and or damage incurred in connection with this tournament. I/we further agree that the Tournament Committee reserves the right to refund this entrance fee if it shall choose for any reason not to accept my/our application. I/we swear that the above facts are true and accurate to the best of my/our knowledge.
